I know this thread is quite old. I've been doing some searches on topics related to adhd and autism. I happen to find a website that may help your family and your brother. They provide useful info on wilderness programs that may help improve the condition of your brother. I am not sure if you have tried it already but I'm pretty sure that one way or another they have helped people who might have the same experiences as yours. Anyway, please read through it.
